Semaglutide. You may have heard of it as Ozempic, Wegovy, or simply "the weight-loss injection." Whatever name you use, behind it stands the same molecule that has changed the way we think about weight control.

In this article we will explain everything you need to know about semaglutide: how it works in the body, how to dose it correctly, what results to expect, and how to minimize side effects.

What is semaglutide?

Semaglutide is a molecule that mimics a hormone your body already produces. The hormone is called GLP-1, and its role is simple: it tells the brain "I'm full, stop eating."

The problem with natural GLP-1 is that it acts for a very short time. The body breaks it down within minutes. Semaglutide is a modified version of GLP-1 that remains active in the body for an entire week. That's why it is injected only once every 7 days.

When you take semaglutide, it does several things at once:

  • Tells the brain "I'm full" much more strongly and for much longer than natural GLP-1
  • Slows down stomach emptying, so after eating you feel full for hours
  • Stabilizes blood sugar; fewer spikes and drops mean fewer hunger episodes
  • Reduces unhealthy cravings for sweet and fatty foods

The result: you eat less without struggling. You don't count calories, you don't fight hunger. You simply don't feel like eating as much.

What is the origin of semaglutide?

Semaglutide was developed by the Danish pharmaceutical company Novo Nordisk. It was initially approved for the treatment of type 2 diabetes (under the name Ozempic, 2017). Later, when studies showed significant weight loss in patients, it was also approved specifically for weight loss (under the name Wegovy, 2021).

At present, semaglutide is the most studied and most widely used peptide for weight control in the world. Millions of people use it. It is approved by both the FDA (United States) and the EMA (Europe).

What results does it show?

Here is what the large clinical trials (the STEP program, with thousands of participants) show:

  • Average weight loss: 12-15% of initial body weight over approximately 68 weeks (one year and four months)
  • For a person weighing 100 kg, this means 12-15 kg
  • About 1 in 3 participants lost more than 20% of their body weight
  • Improvement in blood sugar, cholesterol, and blood pressure
  • Reduced risk of cardiovascular events (demonstrated in the SELECT trial)

When will I see results?

Here is a sample timeline, based on the experience of most users:

  • Week 1-2: Reduced appetite. This is the first thing you notice. Your portions naturally become smaller.
  • Week 2-4: The first kilograms start to go. Usually 1-2 kg, mainly from reduced water retention and reduced eating.
  • Month 2-3: Steady weight loss, about 1-2 kg per month. The body gets used to smaller portions.
  • Month 3-6: The most active period of loss. This is when most people see the biggest difference in the mirror.
  • Month 6-12+: Weight continues to decrease, but more slowly. The body reaches a new balance.

Important: slow and steady weight loss is healthier and more sustainable than rapid loss. If you are losing 5 kg per week, something is wrong.

How is semaglutide dosed?

Semaglutide dosing follows a strict titration protocol. You do not start with the full dose. The reason: if you take a high dose from day one, your stomach will not be accustomed to it and you will experience strong nausea.

Here is the standard protocol:

  1. Weeks 1-4: 0.25mg once weekly. This is the adaptation phase. Don't expect significant weight loss here; the goal is for the stomach to adjust.
  2. Weeks 5-8: 0.5mg once weekly. You start to notice reduced appetite more distinctly.
  3. Weeks 9-12: 1.0mg once weekly. By this point most people are already in an active weight-loss phase.
  4. Weeks 13-16: 1.7mg once weekly (if needed).
  5. Weeks 17+: 2.4mg once weekly (maximum dose for weight loss).

Not everyone reaches 2.4mg. Some people achieve their goal at 1.0mg or 1.7mg. Listen to your body. If your appetite is controlled at 1.0mg and your weight is dropping steadily, there is no need to increase further.

How is it injected?

Semaglutide comes in a ready-to-use pen (like an insulin pen). You dial the dose, choose an injection site (abdomen, thigh, or upper arm), click, and it's done. The whole process takes 10 seconds.

The needle is very thin, specially designed for subcutaneous injections. Most people describe the sensation as a "light pinch" that lasts less than a second. After 2-3 times it becomes routine.

Inject on the same day of the week (for example, every Monday). It doesn't need to be at the same hour, but it's good to keep it on the same day for consistency.

Side effects and how to minimize them

Like any active product, semaglutide can have side effects. The good news: most are mild and resolve within a few weeks.

Most common (especially at the start):

  • Nausea. The most common effect. It is usually mild and appears within the first 1-3 days after the injection. It resolves after a few weeks as the body adapts.
  • Bloating or a feeling of heaviness in the stomach. Cause: the stomach empties more slowly. Eat smaller portions and chew more slowly.
  • Mild diarrhea or constipation. The gastrointestinal tract is adapting. It usually passes on its own.
  • Reduced appetite to the point of not wanting to eat. This is the effect of the peptide, but it's important to eat regularly, even if just small amounts. Don't skip meals entirely.

How to reduce side effects?

  • Follow the dose-titration protocol. Don't skip steps.
  • Eat smaller, more frequent meals instead of 1-2 large ones.
  • Avoid fatty and heavy foods, especially in the evening.
  • Drink enough water.
  • If nausea is strong at your current dose, stay on it for another 2-4 weeks before increasing.

What should you eat while on semaglutide?

Semaglutide reduces appetite, but what you eat still matters. Here are a few simple tips:

  • Prioritize protein (meat, fish, eggs, legumes). Protein preserves muscle mass while losing weight. The goal is to lose fat, not muscle.
  • Eat vegetables with every meal. Fiber aids digestion and increases satiety.
  • Don't skip meals. Even if you don't feel like eating, have at least a small portion. The body needs nutrients.
  • Drink enough water. Semaglutide slows stomach emptying, and water helps the process.
  • Limit alcohol. The liver is busy metabolizing the peptide, and additional strain is not ideal.

What happens when I stop?

When you stop semaglutide, appetite gradually returns to previous levels. This doesn't happen immediately; the body usually adapts over 4-8 weeks.

The key is what you do while you are on the peptide. If you use the time to build healthier eating habits (smaller portions, more protein, regular meals), you will retain a greater portion of the results. If you simply stop and go back to old habits, the weight will gradually return.

Many people do cycles: 6-12 months of use, then a pause, then again if needed.

What is the difference between Ozempic and Wegovy?
