What is Semaglutide?
Semaglutide is a next-generation GLP-1 receptor agonist developed for body weight management and blood sugar control. It is known under the trade names Ozempic® (for type 2 diabetes) and Wegovy® (for weight management).
The active substance mimics the natural hormone GLP-1, which is produced in the intestines after eating. The difference is that semaglutide is modified to remain active in the body for 7 days, allowing a once-weekly injection.
How it works
Semaglutide binds to GLP-1 receptors in the brain (hypothalamus), stomach and pancreas, triggering several parallel mechanisms:
It reduces appetite and the feeling of hunger by acting on the satiety centres in the brain. Patients report a significantly reduced interest in food.
It slows gastric emptying (gastroparesis), which prolongs the feeling of fullness after eating.
It stimulates insulin secretion when blood sugar is elevated and suppresses glucagon, improving glycaemic control.
Clinical results
In the STEP 1 clinical trial (68 weeks, 1,961 participants), patients receiving semaglutide 2.4 mg lost an average of 14.9% of their body weight compared with 2.4% with placebo.
In STEP 3 (combined with diet and physical activity), average weight loss reached 16% over 68 weeks.
Significant improvement in metabolic markers: HbA1c, triglycerides, LDL cholesterol, waist circumference and blood pressure.
Clinical data indicate a reduced risk of major adverse cardiovascular events (MACE) in patients with obesity.
How it is administered
It is administered subcutaneously once a week, on the same day each week. Inject into the abdomen, thigh or upper arm.
Starting protocol with gradual titration: Weeks 1–4: 0.25 mg/week. Weeks 5–8: 0.50 mg/week. Week 9+: 1.0 mg/week (or according to the protocol).
Available concentrations: 5 mg pen (300 units × 17 µg), 10 mg pen (300 units × 33.3 µg), 15 mg pen (300 units × 50 µg).
Gradually increasing the dose is important for minimising gastrointestinal side effects (nausea, diarrhoea).
