What is Tirzepatide?
Tirzepatide is the first dual incretin agonist of its kind, activating both GLP-1 and GIP receptors. It is available under the trade names Mounjaro® (for type 2 diabetes) and Zepbound® (for weight management).
Its dual mechanism of action distinguishes it from single GLP-1 agonists (such as semaglutide) and explains why it shows a stronger effect on weight loss in direct comparisons.
How it works
GLP-1 receptor activation: reduces appetite, slows gastric emptying and improves insulin secretion, similarly to semaglutide.
GIP receptor activation: an additional metabolic effect that improves fat utilisation, increases the sensitivity of adipose tissue to insulin and may improve bone density.
The combination of the two mechanisms produces a synergistic effect, greater weight loss and better metabolic control than either mechanism alone.
Clinical results
In the SURMOUNT-1 study (72 weeks, 2,539 participants), tirzepatide 15 mg achieved an average weight loss of 22.5%, a record result for the pharmacological treatment of obesity.
More than 1 in 3 participants at the maximum dose lost more than 25% of their body weight.
Significant improvement in HbA1c, blood pressure, triglycerides and LDL cholesterol. In SURPASS-2, tirzepatide was superior to semaglutide across all metabolic measures.
How it is administered
A subcutaneous injection once a week into the abdomen, thigh or upper arm. Rotate injection sites.
Titration: Weeks 1–4: 2.5 mg/week. Weeks 5–8: 5 mg/week. Weeks 9–12: 7.5 mg (optional). Week 13+: 10–15 mg/week.
Available concentrations: 5 mg pen (300 units × 17 µg), 10 mg pen (300 units × 33.3 µg), 15 mg pen (300 units × 50 µg).
Wait at least 10 seconds after injection before removing the needle.
